In Japan, the cost of rehabilitation is regulated. A higher wage earner and a longer period of unemployment mean a higher cost of rehabilitation. The additional amount of rehabilitation costs is always variable, and rehabilitation agencies do not have enough resources to pay for it. The aim of this study is to propose a new strategy to solve this problem. The cost of rehabilitation is categorized into three major costs: medical fee, performance fee, and administrative fee. The main factors affecting the cost of vocational rehabilitation are classified into three types: patient characteristics, treatment contents, and the characteristics of the work. The quantity of workload for each patient can be reduced by increasing the work efficiency and by providing more treatment contents. However, since there are almost no treatments available for patients with a moderate level of disability, it is impossible to reduce the amount of workload for these patients. It is, therefore, necessary to change the classification of the treatment contents and to use the remaining resources more efficiently. The new strategy is as follows. First, the current classification of the treatment contents should be reviewed. Next, the classifications based on the principles of work efficiency and the principle of disability-oriented treatment should be introduced to reduce the amount of workload for patients with a moderate level of disability.
